PLAYER COMMITMENT

"The D6 one league rule has been replaced with a player commitment contracts that reads as follows -

COMMITMENT TO TEAM – Players are expected to participate in all team activities (practices, games, tournaments, etc). An allowance for an “excused absence” upon notice that is reasonable under the circumstances may be established by teams and associations. However, unexcused absence is grounds for consequence to a player, as determined by the team or association, up to and including suspension.

Waiver Policy

Waivers are used to allow a player to play with another association.  The Waiver is valid for only one year...every season, a new form must be submitted for approval.  Waivers are required for ALL age players - Mites are not an exception.

Waiver Procedures

1. Have the Waiver form filled out by the parents and attach a copy of the IMR from the releasing association.  The IMR will state that the player has been registered, transmitted and will give you the infomration to enter the player into your system as Previously Registered.  You will not have to pay any additional fees when the player is transmitted.  DO NOT accept waivers without a copy of the IMR from the releasing association.

2. Submit the form to the Releasing Associatoin President or his designee for approval.  If the player is moving between districts, the Releasing District Director must sign the form before it moves forward.

3. Submit the form to the Accepting Association President or his designee for approval.  If the player is moving between districts, the Accepting District Director must sign the form before it moves forward.

4. All 3 copies of the form MUST be presented for the roster verificaion...No photo copies are allowed with the exception of Multiple Skater Waiver Forms.  Multiple Skater Waiver forms, aka Rainbow Team Waiver forms, must also be submitted with 2 copies of the original signature page.

     - MN Hockey has declared that these types of waivers only need to be  obtained the first year they are requested there for you do not need to get another signed. Copies are on files with D6 and JHBC and acknowledged by KHBC.

     - During the first year of transfer, the skater cannot make an A team.  They must either sit out that year or make the B team or below.

Non-US Citizens

 

For all non-US citizens, a form is required to be completed. There are two forms: one for Canada and one for contries other than Canada. The forms are found in Cybersport directory USAH/Cyber 11-12/;USAH Forms and are titled Written Transfer Canadian and Written Transfer International. Once the form is completed, it is to be mailed to the address at the bottom of the form and it will be returned when the transfer has been approved.
